- Four home care agencies were named to the 2013 HomeCare Elite list compiled by Outcome Concept
Systems, Inc., a Seattle-based company providing data products and benchmark services for home
health, hospice and private duty agencies. The listing of the top tier U.S. home health care agencies
honors those that rank in the top 25 percent in their region based upon quality of care, quality
improvement and financial performance
- Acquired a hospice license for the Southern California Region serving Glendale Adventist Medical
Center, White Memorial Medical Center and Simi Valley Hospital
- Increased admissions in 2013 for home health and hospice by three percent as compared to 2012
- Implemented a new customer management software to assist with the promotion of services and
expertise of the home care service line to the medical community, referral sources and community
service groups
- Increased efficiency and accuracy of assessments by centralizing quality assurance and coding
- Implemented medication management in our hospice locations, reducing costs while ensuring
patient comfort
- Continued to develop programs to meet national health care priorities to reduce re-hospitalization
and improve the health of those with chronic conditions
